Bal-Lance Salon a complies with all State Board of Cosmetology Rules and Regulations, and the guidelines of the California Department of Public Health (CDPH), Occupational Safety and Health Administration (OSHA), Centers for Disease Control and Prevention (CDC) and Barbicide.
* I agree that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the last 30 days.
* I agree that all household members have not shown any signs of a fever, cold, cough, shortness of breath, chills, muscle pain, sore throat, or recent loss of taste or smell within the last 14 days. All household members do not have a current order to self-quarantine.
* I do not have pending COVID-19 test results. If I have pending COVID-19 results, I will inform Bal-Lance Salon prior to my scheduled appointment. I understand that Bal-Lance Salon has the right to reschedule my appointment until I receive negative test results, or have fully complied with an order to self-quarantine.
