* I agree that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the last 30 days.
* I agree that all household members have not shown any signs of a fever, cold, cough, shortness of breath, chills, muscle pain, sore throat, or recent loss of taste or smell within the last 14 days. All household members do not have a current order to self-quarantine.
* I do not have pending COVID-19 test results. If I have pending COVID-19 results, I will inform Bal-Lance Salon prior to my scheduled appointment. I understand that Bal-Lance Salon has the right to reschedule my appointment until I receive negative test results, or have fully complied with an order to self-quarantine.
* I agree to take a health screen test with a no contact thermometer before entering Bal-Lance Salon.

* Mandatory Salon Forms
All reservations REQUIRE a “2022"New Guest Information & COVID Liability"” to be completed BEFORE entering the salon.
* Under the age of 18
A Legal Guardian must consent in person with the Artist before the service can begin and fill out required forms as well. If the service is unable to be complete due too safety, fee with still apply.
* Credit Card/ Deposit
A Credit Card capture is required for all services. All Chemical Services (Color, Lightening or Smoothing Treatment) require a 20% non-refundable deposit, which goes towards your services.
STRICT ENFORCED CANCELLATION POLICY
Things come up all the time, we understand totally. This is why we message you 48 hours in advance because we know how easy it is to forget an appointment you booked. Since the services are reserved for you personally, a 25% non-refundable deposit that goes towards your appointment will be required for all services.
Less 48 hour notice a 20% fee will apply, NO EXCEPTIONS!!
Less than 32 hour Cancelation notice will result in a 50% charge fee of the reserved service amount, NO EXCEPTIONS!!
"NO SHOWS" will be charged 100% of the reserved service amount, NO EXCEPTIONS!!
Since the service and time is reserved just for you personally, we respectfully request at least 48 hours notice for Cancellations. Less than 32-hour cancellation notice will result in a 50% charge of the reserved service and time. "NO SHOWS” will be charged 100% of the reserved service and timing blocked personal for you. For the inconvenience of the "NO SHOWS" a 10% Gratuity will also be added to the "NO SHOWS" of the reserved service and timing blocked personal for you.
Due to Covid-19 Safety Measure we may/will need to reschedule your reservation if you arrive 15 minutes after your scheduled appointment time. Therefore, “NO SHOW” your reservation.
* SERVICE RE-DO
Your happiness is our goal! Should you have any issues with your cut or color(non-corrective), let us know within 7 days of your visit and we will happily fix your color or cut.
* PRODUCT EXCHANGE/ RETURN
Should you make a retail purchase that you are not satisfied with, please bring in your product within 14 days of purchase and you will be issued an exchange or full refund.
* CONFIRMATION TEXT
As a courtesy, we will text and confirm your service appointments 48 hours prior and 24 hours prior to your appointment date. However, if we are unable to reach you, and can only leave a message, please understand that it is your responsibility to remember your appointment dates and times to avoid late arrivals, missed appointments and the cancellation fee.
